My private pay rates are listed above. Payment is due at the time of appointments. This allows more in-depth, flexible, thorough and personalized treatment. I will provide a superbill upon request, to utilize for out-of-network reimbursement.
ACH payments, Zelle, Venmo, HSA/FSA cards are accepted.
For insurance coverage, I only accept Anthem Blue Cross and Blue Shield New York, via Headway. We will utilize Headway to bill this insurance, and you will need a Headway account.
I do not currently participate with any other insurance companies.
